Appropriate study instruments will be developed that will capture the data required to report on the outcome measures listed above. As mentioned earlier, for programme performance indicators, we will use the pre-defined programme database as the main source of data hence no study instrument will be developed for this purpose. Instead, we will take time to verify existing use of the programme database to ensure that all necessary information is being collected appropriately.

Study Instruments for Incidence Study

Following are the various study instruments that will be used for the incidence sub-study:

Form Name Form Code Download
Screening Form Form A
Consent Form Form B
Recruitment Form Form C
Follow-up Form Form D
Follow-up Tracking Sheet Form E
OTP Referral Form Form F
TSFP Referral Form Form G

Study Instruments for Stepped Wedge Cluster Randomised Study

Following are the various study instruments that will be used for the main stepped wedge cluster randomised study:

Form Name Form Code Download
Mother and Child Form – Baseline Form 1
Mother and Child Form – Rollout Form 2
Pregnant and Lactating Women Form – Baseline Form 3
Pregnant and Lactating Women Form – Rollout Form 4

The forms above can be used if the study is to be conducted using the pen and paper interviewing (PAPI) method. For this, the above forms should be printed and administered based on the study protocol described here. Once paper forms are filled out, data should be double-entered into a computer using EpiData, cleaned, compiled and managed using the data management protocol specified here.

An alternative to this approach would be the use of computer-assisted personal interviewing (CAPI) method using a mobile computing device such as a mobile phone or a tablet device. For this survey, we have decided to use Open Data Kit (ODK) as our tool for implementing the CAPI method using tablet devices running Android operating software. Based on the specified protocols for administering the PAPI-based forms, we authored and developed an XML-based version that combined all of the paper-based forms into a single comprehensive form with appropriate skip logic and conditional branching that automatically pointed the enumerator to the appropriate next form based on the responses from the respondent. The XML form or XForms is then uploaded to a cloud-based server, a local area server and onto tablet devices for use in the data collection for the study.

The CAPI-based study instrument can be viewed online through a web client (i.e., web browser) by clicking on the following links:

The CAPI-based study instrument can also be viewed using a mobile client (i.e., mobile phone or tablet device) running Android operating software and with the ODK Collect application installed. Instructions on how to access the questionnaire through a mobile client can be found here.
